Recommendations and Documentation for Incident Investigation
This video discusses the importance of clear communication and documentation of recommendations for corrective actions following an incident investigation. Provide clear descriptions, including names and positions of those responsible for implementing the recommended corrective actions and include completion dates for when the changes should be implemented. Action items should also be prioritized. Communication of the documented recommendation can either be a formal Investigation report with limited distribution or a widely-distributed flyer communicating the contributing factors of the incident and chief lessons learned. Regardless of how the information is conveyed there are important considerations to be made. Always include a description of what happened. Think about who needs to know this information. Only disclose this information to people who will benefit from it. Then decide how to communicate the information most effectively. Communicating information effectively is an important step to making sure incidents to not reoccur.
